Pathfinder Program Details

Our team works with each client to increase self-determination and independence through a focus on translating insight into action amid real-life experiences. It’s a person-centered approach that keeps everyone involved in the participant’s care focused and motivated to achieve goals and move toward an individualized concept of “a life worth living.”

Hallmarks of the Program

  • Focus on psychosocial rehabilitation
  • Community living in local apartments, with 24/7 in-apartment support from our trained resident advisors 
  • Opportunity to transition to day program while living independently outside of the mentored-apartment setting
  • Individual recovery plans
  • Average stays are three- to six-months to provide adequate time to develop and master skills and competence in daily living, although there is no minimum time commitment after the first month
  • Individualized educational assistance, including academic and readiness assessments, degree and career planning, tutoring and help with class registration and selection
  • Supported-employment model to help clients with vocational assessment, counseling and goals and to explore career development with career coaching, résumé development and job searches
  • Internship, work or volunteer experiences
  • Group and individual wellness, recovery and social activities
  • Group leisure and therapeutic outings, including concerts, festivals and professional sporting events
  • Programming that addresses life and interpersonal skills; group psychotherapy; dialectical behavioral therapy; acceptance and commitment therapy; mentalization-based therapy; and cognitive behavioral therapy
  • Weekly individual and family therapy and case management as well as specific support for individuals with eating disorders, substance abuse and anger management issues
